Well, it is taken directly from the episode. But I forgot why exactly he is smiling. Definitely have not watched it enough yet. maybe someone can serve with the answer before I get the chance? Unfortunately I have to work

It's the mment when Sherlock tells John that once he'll excrete the drug everything will be fine. And John answers that he's taken care of that already.

For me, this moment is THE highlight in series 1:
John, nodding the answer to a question Sherlock hasn't even asked him.

Yes indeed. What a beautiful yet horrifying moment.
When he sees John at the pool there is also the this tension right before he sais John's name.
But the sound cannot be captured in a picture. This scene makes me choke up every time.
