I also had an awful thought about the nurses in the trailer, and the

children's hospital

from setlock - that Jimmy Saville had such a lot to do with hospitals in such a sinister way.    I'm not sure they'd go as far as Saville's extreme depravity, but who knows?

It definitely sounds like a reference to Jimmy Saville and they did say Culverton was the scariest villain yet so who knows indeed.

Or he's not under influence at all just like in the original story.
At least in the beginning, when he hopes to outwit Smith.
Later on Sherlock looks more scared than in HOB, so...

the preview makes is sound as if the drug thing was voluntary, but we know in canon Sherlock fakes his disposition, so I'd say both is very much possible. Sounds interesting anyway

Good points, all.  It could go either way!  I can see Sherlock taking drugs in grief, but actually in that scene with Mrs Hudson, he doesn't seem to be going down that route.  He's looking for a case, half-heartedly, and Mary gives him one.   I think the importance of saving John would tend to keep him off the drugs.   So you could be right that the illness Sherlock would fake would be drug addiction.

We've also seen him drugged involuntarily in the past.

I think it's fabulous that Mycroft is targeted! Firstly, we'll probably get to know more about him. Secondly, I'd love to see Sherlock rushing to the rescue for a change. So far we've only seen Mycroft caring about his brother time and again even tho both love each other dearly.
