A well lit room, dark clothing, blur (Or Vaseline on lens/filter) examinatory stance suggesting prominent part, I'm going with Mycroft's first appearence during 'A Study in Pink'.

No. Mycroft's first appearence happened in a dark room. On this picture it's really bright, it's outside. You could tell by the car in the background.

I don't mean Sherlock himself, I mean when he's in that dreamlike floating state as he's heading back to 221b having heard Irene and John speak about him in that warehouse. He's about to see that Mrs Hudson is in danger and sharpen up.

It's the blur that's throwing me. it's overdone for a regular shot. We're needing dream-like qualities about it.

Well this will have to be final guess from me as I don't recall the episode as well I I had having watched it so Sherlock and John in the car leaving Buckingham Palace and discussing they're next moves, this is a character from outside of the cab.
