I think you have your answers right there - he as so high he was weeks from dying (and even Bill Wiggins said he had gotten too much), and she was in disguise. 
Also, I can imagine that it has been many years since Sherlock saw her. She might have changed.

He couldn't make any deduction in her, he hasn't seen her for a long time and she's just as smart as any other Holmes breed. She could figure out to be not noticed

Maybe she did something to Redbeard. Maybe there is some explanation why Mycroft keeps mentioning Redbeard.
One thing I am sure of: the next episode will be very interesting. Finally some answers?

I think he may have suppressed her completely due to some trauma. And the memories are slowly creeping in since he started allowing emotions.

I'm so confused about this... if their sister's name literally means "The East Wind"... and Sherlock said on the tarmac that "The East Wind takes us all in the end"... He never made the association with his sister? Or he already knew she was going to show?

Did anyone else recognise her from the start?  I had no idea she would be Sherrinford until just before she announced it, but I'd written a note at the beginning - 'the therapist is Sian!'.   Was waiting the whole time for her to be revealed!  I liked that they used disguises in this show. 
Ewige - the subtitles spelt it Eurus, I think.
