I'm wondering if, for this particular scene, he is faking it. Seeing as he walks into the ambulance on his own, and neither John nor Molly follows him. Compare this with HLV, when he got shot, John was very worried and rode with him in the ambulance. Here, John has the time to turn away and talk to someone else, before heading for the limo (at least not towards the ambulance). And Molly walks off as she's catching a bus or something.

However, why would Molly be in her lab coat in a house? She doesn't do field work, that is Anderson's department?

I agree that it's odd, Vhanja.  And there is nobody else with him either, paramedics or whatever.  It doesn't make sense to just park outside, open the doors and wait for somebody to get in.  It could be that Sherlock is setting up a scene (e.g. he plans to arrive at the hospital in the ambulance as an "emergency"), which would explain his look but not his gait.  It could be that he has taken drugs to make him seem more ill and fool the hospital staff, perhaps, and we are seeing some of the effects.  But then again, if he was trying to set up that scene, then it would make sense to have him carried into the ambulance for show at least.

Ok, so let's go for headcanon, then:

It's John and Mary's house. Sherlock is visiting. Wearing his dressing gown and 5 o'clock shadow "for a case, John". Molly drops by straight from work, because Sherlock asked her to. He then goes on about a case, Molly needs to help and if John can't go to Mycroft to ask him to do X and Y? 

They are all off to to their task when Mary goes: "But you can't go, John, what about little Pink Baby Watson?" And John turns around, yells "I don't care!" and runs off to do what Sherlock asked him to do.

Btw, I just saw a Benedict pic from today at Hay Festival. Still unshaven. So this scruffy style will probably go on next week.

Ah, so that answers y question earlier - if the unshaven look is make-up or them asking the actors to not shave.

Liberty - I agree, if this was real, at the very least, paramedics would be there to support him on his way inside the ambulance.

Sorry, I crossposted with you above, Vhanja.  Yes, it could be.  I wonder if John and Molly could be there because they're both involved in faking Sherlock's illness (both have skills which might be useful).  (As you can see, I'm quite taken by the faked illness idea).  Molly might be wearing her coat from work as you said (yuk!) or wearing a coat because she's "working". 

I'm still intrigued by Mrs Hudson being there and the comments about '70s decor!   That could mean mind palace.

It is not the Watson house, at least not the one we know. It really does not look like as if they were living there. People who had a peak said it was furnished sort of retro style. There is also an unknown woman who seemed to be in the house at one point. And the baby has completely disappeared since episode 1.

Hm, well, usually they would use a real house/building for the exterior and then a set dressing for interior, wouldnt' they?

It doesn't make sense that Sherlock would be doing drugs again. At the end of TAB, he is back in England, got a great case going with leads (Moriarty) and John - so why would he use drugs?

Yeah, but for some dark haired men, it doesn't take them long to get the 5 o'clock shadow and if he's not washing, or looking after his appearance etc...but anyway I assume he would have a 'top up' to fake illness.
It makes sense though if the Watsons argue about John going and would then explain the shot we've seen where he says to Mary: what are you doing here?
If she's gone after him.

But this is episode 2. He was clean shaven in episode 1.

I think you're right about interior and exterior shots, but the strange thing is that these houses don't look to me like they existed in the '70s (please correct me if I'm wrong - not an architectural expert!).   It seems unlikely that the house would just happen to have '70s decor if it wasn't built then.   Even if it was a new built that early, it's quite a long time ago - it's really unusual to see obvious '70s decor now.    It's 40 years ago .... even if an elderly people in their 70s or 80s were living there, that would mean that they hadn't updated the decor since their 30s or 40s.    Although it's possible that the current occupants have a room or two deliberately decorated in a retro '70s style, rather than the decor just not changing. 

As for going straight from the plane .... I don't think so, but it's not impossible that they would flash back to that point in Ep2.    Still doesn't really explain the ambulance, etc!   Ooh, this is exciting!

Someone brought up the idea that the grey-haired woman between 40 and 50 seen on set could be Harry Watson. I really would like to meet her after all that time.
