Oh, I didn't think of that, Tonnarree. A dressing gown would make sense if he was filiming something partially nude (!), like the previous hospital scenes, for instance (but I keep thinking it's a children's ward, so he's clearly not a patient on this particular ward - could be a patient somewhere else, and having flashbacks or mind palace, etc.). I was just thinking that it was so iconically Sherlock that it had to be a Sherlock dressing gown! (Maybe he's had to rush somewhere in the night?). I'm liking Nakahara's idea of Culverton Smith - maybe Sherlock could be "dying" (again)? I think I'm preferring the idea of a nude scene at the moment, though.

Who knows what could be going on. I certainly don't have any ideas on what he could be wearing in a scene like that, unless he spent the night at the hospital with Mary and John or something like that.
Unless it was Benedict in between costumes?

There is much discussion going on about them using elements of "The Dying Detective". So Sherlock might pretend to be a patient in a hospital to investigate something. He is wearing his own sweat pants, not a costume. He probably changed into a pyjama on set and put on the dressing gown.
This is also supported by the use of the name "Gedgrave" on a hospital sign since the name is from the above story. So Toby Jones might be Culverton Smith, the villain featuring in this story.

I'm loving all these tidbits! I'm presuming the use of Gedgrave as a hospital name means that it won't be used as a character name, so we won't see that actual character. If it's a children's ward, then the original victim might be a child (baby Watson?). It still seems odd that Sherlock would be a patient in a children's ward, but there could be another explanation (such as them moving the set around for different takes). I'm aware that they do tend to mix up bits and pieces of different stories and make new ones, so it's unlikely that it will follow a particular story to the letter.
