Hm, if CAM is back, it would have to be flashback. He was shot in the head on screen (although it is possible to survive a shot to the head, depending on where you hit). It doesn't seem as if Sherlock has a history with CAM before HLV (it seems as if that is the first time they meet).
My first thought was with Mary. It seems they have had some communication before.

I'd be very happy with that, too, but I don't know if everyone will be and it would as confusing as hell for new viewers.
I sat in the cinema at TAB and thought god, I hope it's all fanatics in here, cos this is complex!
My colleague waltzed up to me at work after TEH was first shown and said: well I tried to watch that show you're mad on, and couldn't make head nor tale of it!
I did point out to her, she was being a tad unreasonable.

As far as the S4 being confusing for new viewers, The creators can't write for new viewers. They have a narrative arc they are committed to and limited time to complete it.
It's foolish to start a series of any kind in the middle. Especially something as complex as Sherlock. I always tell people to start at the beginning.

True. And it is not just the narrative but also the constant references - verbal, visual, etc. - to older episodes. "It didn't work for me." "How would you know?" You can only appreciate the complexity of the show if you have watched it from Pilot to TAB.
