Very interesting read. And she has some good points. I've always figured all the gay/couple assumptions are just meta-jokes from Moftiss, because you have to deal with the issue one way or another (with two single grown men living together in this day and age).
Also; I figured that the reason Sherlock is vague in letting down John (not really stating his sexuality one way or the other) was that at that point Moftiss wasn't really sure themselves what sexuality Sherlock had. So they kept all options open, so to speak.
(Her nick sounds familiar, though - does she write Johnlock fanfics?)

I think it's true that Sherlock's relationship with John was an exception right from the beginning, and that other people notice that. It is supposed to be a special relationship, but even without that particular bond that they have, I don't think John is one in a series of friends/flatmates/partners that Sherlock has had. This is a first and only relationship for Sherlock.
However, I think Angelo just makes assumptions in the way that people do. That's played on in other parts of the series. Two guys living together - gay. Two guys going on holiday together - gay. Two guys going out for a meal together - gay. I think it's played up in the series, but it happens in real life too (I know I'm guilty of it). I don't think any of these people have seen Sherlock (or John) actually dating men.
I also think it sets up Sherlock's position in regard to dating, while leaving his sexuality ambiguous. I think his sexuality is irrelevant to him at that point ... and he doesn't try to put John off by saying that he's not gay, because that wouldn't actually be the reason (whether he's gay or not). "Married to my work" is the reason, and that's elaborated on more in ASiB, I think. But it's true. The only time we see Sherlock apparently dating (Janine), it turns out that it's purely work.
(One thing I think gets missed is that the reason for them going to that restaurant is so that Sherlock can watch for the killer. It looks like a date to Angelo, and obviously to a lot of viewers - but that wasn't the intended purpose. And it wouldn't make sense for Sherlock to take John on a "date", then tell him he's not interested.)

It looks as if he just hasn't ticked any of the boxes. I don't honestly think it's trying to tell us he's gay or bi (they could just have shown him dating men, and this is such a small detail it will be missed by most). There are other reasons for not giving blood, and in fact, most people in the UK don't.
Nice find, though. I do love when people look at the details like this.

Of course, one might see it as an uncompleted form -though I can't picture John not completing an important medical form. That's why I'd rathern interpret it as a Johnlock-y sign

Sure, I can understand why it's being interpreted that way.
And he does seem to have put a line instead of a signature or address, so it's not necessarily an uncompleted form - just not wanting to be added to the register, for whatever reason. He also might have ticked the box at the end. But I wouldn't assume that the 96% of people in the UK who don't give blood regularly are gay . There are much more effective ways to get that message across if that was intended.
