Yeah, I think the violin thing is definitley a stand in for Ben, but I should imagine it will be more focus on his hands and the actual playing, rather than any long shots. If they were doing long shots and even the back of the head was in view, then he would have to look sufficiently like Benedict. If it was just arms and hands on a close up of the violin when he was playing, then it wouldn't matter a bit what he looked like. They need to know the measurements so that shadows can match up when they're filming. If someone was really short and didn't match, they'd have to stand him on a box for example, and things get even more awkward if people are too tall. Waist and chest size - they probably have a decent stock of black suits and purple shirts all ready to go (and all in Benedict's size), so it's much simpler if whoever they cast can fit into those.
Productions are ALWAYS trying to make cut backs on the budget, especially the BBC and I should imagine, especially Sherlock, because they spend so much money on great cinematography and specialised set up shots that if they can save on other small thins, so much the better. Extras have to provide their own costume the majority of the time, this is a common thing throughout all the industry, and it's also perfectly normal for them to only send out the casting call a few days before the shoot. Doesn't necessarily mean that they have suddenly decided to change or add something. If you think about it, it's probably much harder to get mime artists than people who have access to a police uniform, so that's why they needed to advertise sooner for that one.
