I was also really confused by the little finger but I think it's a cane with the word "Oh..." from when John realises that he forgot his cane...

Imagined a similar interaction between the two, even though more likely he doesn't contact anyone, and picture Molly just trying to be there as a friend anyway. Awwww....

I thought that, too and maybe the whip referred to Irene + Irene's personalised ringtone for Sherlock went something like 'Oooh'
