I'm just thinking... maybe the nurse is really Mary, Sherlock thinks he's hallucinating, John thinks Sherlock was the one behind it and this is why he punches Sherlock?

I'm not sure. He also has a blue suit in the other publicity photo, but no pocket square.
There was a hint ages ago about a TV presenter or show, and I think some of these stills are what we are seeing.
. I had wondered if the TV aspect was going to be mind palace, but seeing the Business Killer poster on the bus stop in TST suggests that it's real. I think one of the interviews said he was a businessman, so perhaps a businessman who presents a TV show, like Dragon's Den. (I had a wild leap a while ago when I realised that the US version of Dragon's Den is called Shark Tank, and we were getting all the shark hints ...), or a chat show about businesses. This would be my guess. I'm not sure why Mycroft would be on that sort of show, so I'm guessing it's not him for now.There was also a tiny snippet of script leaked, which hinted at the same thing

Just realised that it would be silly of Mycroft to appear on a TV show... Not very sensible for the British Government.... So, maybe Smith is the guest in a show and the guy on the left is the presenter...

I think you might be right, Pops. It looks like he's a judge in the red button picture, but that he's being interviewed in the other picture. It looks like TV is going to be a theme for this episode. Interesting, because I loved Magnussen and the newspaper theme.
There's also another picture with Culverton and Sherlock where it looks as if they're being followed by reporters - or possibly still filming a TV show?

A friend of mine has this crazy theory that Sherlock will have to kill Mycroft in order to save John. That would be consistent with Mary's "go to hell" comment and the flying chess pieces also indicate a kind of a stand-off between the brothers.
