Isn't Sherlock famous? The scene in the LR of 221b, the one with the death frisbee deerstalker hat, John was sitting on the couch, reading about them in the tabloids. So it isn't necessarily a mistake that people (especially girls, lol) might turn to look at them as they walk by in public. That distinctive mop of hair and that coat-- those two things alone would make him highly recognizable. For all we know, those extras were supposed to turn and look at them like that.
By ASiB Sherlock is midly famous and so is John (at some extent), but at the time of TBB they weren't so well known. That said, people do stare without reason in real life, so it's not so strange that some people stared at two good looking guys like Sherlock and John.

This - and there must have been all the camera stuff and people around. Who wonders that persons are trying to find out who are the objects of being shot ? I would, at least, whether I know the actors or not. That´s always exciting (and a risk of open air shoots).
